Near miss report form
A near miss is an event that did not cause harm but could have, and it is the best warning you will get before a real incident. This short form records what nearly happened, where and why, so the cause can be fixed while it is still cheap. It is a different form from an incident report because the stakes and the tone are different: there is no injury to describe, only a lesson to capture. The worked example records a ladder that shifted during a stock check. Recording a near miss costs nothing; ignoring one is how the same event becomes an incident.

1. WHAT A NEAR MISS IS
> Note — Why recording the almost-event matters.
A near miss is an event that did not cause harm but could have, under slightly different conditions. It is recorded so the cause can be fixed before the same event causes harm. The presence of a near-miss record is how a workplace finds its problems while they are still free to fix.

Stockroom, high shelving, back wall.
During a stock check, a step ladder shifted on a slightly uneven floor tile while someone was on the third step. The person stepped down safely and no one was hurt.
The cause
The ladder was on a tile that rocks, and the ladder had no locking feet to grip it.
What was done
The ladder was replaced with one that has locking feet and a wider base, and the uneven tile was marked for repair. The owner confirmed both within a week.

Why near misses are worth recording
A near miss is an event that did not cause harm but could have. It is the best warning you will get before a real incident, and the cause is still cheap to fix. This is a different form from an incident report because the stakes are different: there is no injury to describe, only a lesson to capture.
Turn a folder of documents into something a team can actually find and follow
Every template on this site is free and stays free. The kit is what turns a folder of documents into a system you can actually run: a register with numbers, owners and review dates; who reviews what and how often; a numbering scheme that still works at fifty documents; and the thirty-day sequence for getting a team to use any of it, which is where most of these projects die.
